General Instructions:
1. Describes the student’s performance using the Rubric’s scale below
2. Calculate the evaluation grade using the formula below. GRADE: EXTENSION:

Rubrics Scale:
5 – Outstanding behavior or Performance without supportive cues
4 – Very satisfactory behavior or performance with occasional supportive cues
3 – Satisfactory behavior or performance with regular directive cues in addition to
supportive cues
2 – Needs improvement or performance with frequent directive cues
1 – unsatisfactory behavior of performance with continual verbal and physical directive cues

CRITERIA ROTATION
KNOWLEDGE (15%)
1. Identifies relevant patient information and the signed consent to treatment or procedures.
2. Defines the disease process, signs and symptoms, and causes.
3. Recognizes the affected organs and/or systems.
4. States the mode of transmission of an actual or potential infectious agent in the care of the patient.
5. Enumerates the pertinent information regarding medications of the patient including nursing
responsibilities
6. Names laboratory tests, and diagnostic procedures in the shift for the patient.
COGNITIVE (15%)
1. Explains the disease process in the patient.
2. Interprets the treatment or management plan that includes laboratory and diagnostic reports, medication
orders, and procedures.
3. Formulates and prioritizes nursing diagnosis
4. Plans nursing activities according to priority.
5. Differentiates the chief complaint and other history relevant to the case of the patient.
6. Justifies the plan of care based on ongoing patient assessment or outcome evaluation.
INTERPERSONAL SKILLS AND RESPONSIBILITY (15%)
1. Demonstrates respect to patient's rights that includes privacy and confidentiality of care and information.
2.Shows punctuality, diligence, and good working relationship among health care team members, clinical
instructors and peers.
3. Uses complete uniform and paraphernalia.
4. Writes requirements on a timely manner.
5. Illustrates proper referral and collaborative efforts for patient care.
6. Gives health teachings based on health needs of the patient.
COMMUNICATION, INFORMATION TECHNOLOGY AND NUMERICAL (15%)
1. Demonstrates skills in History taking, including data gathering, data processing and communication.
2. Calculates drug dosage, intravenous fluid infusions, anthropometric indices (e.g., body mass index or BMI, body
circumferences), hemodynamic parameters, risk estimates (e.g., Braden Scale, Morse Fall Scale, Glasgow Coma
Scale, Acute Physiology and Chronic Health Evaluation or APACHE, Thrombolysis in Myocardial Infarction or TIMI
Score) intake and output, nutritional requirements, obstetrical measurements, functional ability or acuity scale
and staffing requirements, etc. accurately.
3. Assesses the biopsychosocial-spiritual needs of the patient using verbal or subjective cues and nonverbal or
objective cues keenly.
4. Appraises the plan of care with health care team members and Nursing Instructor.
5. Evaluates patient outcomes with Nursing Instructor.
6. Illustrates verbal and written handoffs or documentation to transfer care according to agency practice.
PSYCHOMOTOR (40%)
1. Performs physical assessment correctly using clinical check-off or flow sheets.
2. Operates specialized medical devices and various types of equipment or contraptions as indicated and/or
electronic health records software or healthcare database proficiently.
3. Demonstrates skills to perform planned clinical procedures towards holistic patient care and aftercare of
equipment and management of supplies or agency resources.
4. Employs international patient safety goals or IPSG that includes infection control protocols using standard
and/or transmission-based precautions (e.g., hand hygiene, appropriate personal protective equipment or PPE)
and disposal of health care wastes (e.g., infectious, pathological, sharps, chemicals, pharmaceuticals,
genotoxic, radioactive, non-hazardous or general wastes).
5. Employs therapeutic communication in nurse-patient interaction or "nurse presence" in high acuity or long-term
care.
