1 Have the correct material been used ?

Have the site operatives conducting the pour been instructed what needs to be
2 done to ensure that the design concrete pressure is not exceeded during the
poure?

3 Are the shuttering material such as bolts nuts tie rods and other fasteners of the
correct approved design grade strength?

4 Is the formwork laid out generally as per the manufacturer design


specification?

5 Are the backing members the correct type, spacing and position with joint details
positioned and made as shown?

6 Are the ties the right type, diameter positioned as shown and are the correct
tie plates and nuts used? Are they seated correctly?
Are the alignment props or Tie rods of the correct type, positioned and
7 adjusted correctly, adequately connected at the top and anchored at the base.

Are all of the stop-ends in place with weging, propping, strutting and other details

shown in manufacturer design specification.

Is the access and working platform that has been erected safe and is there safe
9
methods of access and egress?

10 Have all notes on the manufacturer's specification complied with?
