Porac, is serving in a Geographically Isolated and Disadvantaged Area
(GIDA) under the jurisdiction of the Schools Division of Pampanga.

Said school/workstation is included in the recognized list of GIDA areas, as


defined by applicable government issuances and guidelines.

This certification is issued upon the request of the above-named personnel


for purposes of complying with the requirements for Individual Availment –
Payment of Medical Expenses under DepEd Order No. 16, s. 2025, in relation
to the grant of the Php 7,000.00 medical allowance to eligible personnel.
