1. In some of bunker houses, toilet seats & flushes are damaged & bathroom bulbs are fused & hence need immediate replacement. 2. Mosquito net must be immediately fixed at all windows of kitchen room, dinning hall & general toilets & bathrooms. 3. The door with door closure need to be fitted at kitchen room & dinning hall.

MAJOR SITE SAFETY DEVIATIONS


1. The Escape device is out of order due to broken seat belt. 2. The pig house is fitted with non- flammable proof fitting. 3. The illegal wire to wire connection on sensing device fitted on shale shaker tank. 4. The diesel tank earthing need to be separated.

5. One earthing loop missing in Power Pack. 6. 6 motors are running with single earthing. 7. Some floor openings are present on mud tanks. 8. Poor boy working without pulley guard. 9. Improper access is provided for water pumps. 10. The position of eye wash fountain near Hopper area need to change. 11. The water hoses are lying on mud tanks possesses serious tripping hazard. 12. The G.I. pipe is running above ground. 13. Hearing protection need to provide during working at / near high noise area. 14. The general house-keeping need to improve.
