ASSISTING THE PATIENT IN ACTIVITY OF DAILY LIVING

TOPIC;ASSIST CLIENT IN ACTIVITY OF DAILY LIVING

SUBTOPICS-
-Assisting the client meet elimination needs,
-assist client take medication,
-observe unexpected behavior and report.
- measuring and recording height, weight, body temperature,-
observe abnormal breathing and report to supervisor,support patient on oxygen,reposition
prongs , general precautions for the safe use of oxygen

1. Assisting the Client in Meeting Elimination Needs

 Assist clients to the bathroom or with the use of bedpans, urinals, or commodes.
 Provide perineal care to maintain hygiene and prevent infections.examples use of tissues
 Monitor and document urine and stool output if required inorder to note any deviations
from normal
 Recognize and report any abnormalities (e.g., blood in urine, diarrhea, constipation).

2. Assisting Clients in Taking Medication

 Ensure medications are taken as prescribed by observing the five rights of a patient
during drug administration,right drug. Right drug , right administration, right route . right
dose
 Offer water or food if required with medication.
 Observe for side effects or adverse reaction and take actions immediately

 Report any refusal, missed doses, or unexpected reactions to a supervisor.

3. Observing and Reporting Unexpected Behavior

 Monitor for confusion, agitation, aggression, or changes in mood.


 Note any unusual fatigue, dizziness, or restlessness.
 Report significant changes promptly to a healthcare provider or supervisor.

4. Measuring and Recording Vital Signs

 Height & Weight: Track changes over time for overall health assessment.so as to
identify malnutrition, overweight

Body Temperature: Measure using a thermometer and report fever (above 100.4°F or
38°C).Blood pressure , Blood pressure is measured in millimeters of mercury (mmHg) and
consists of two numbers:

1. Systolic (Top Number) – Pressure in the arteries when the heart beats.
2. Diastolic (Bottom Number) – Pressure in the arteries when the heart rests between beats.

Normal 90-120 60-80

Elevated 120-129 Less than 80

Pulse rate,

Tachycardia (Too Fast): Above 100 BPM in adults


Bradycardia (Too Slow): Below 60 BPM in adults (unless an athlete)

5. Observing Abnormal Breathing and Reporting

 Watch for difficulty breathing, shortness of breath, or irregular respiration.


 Check for signs of respiratory distress (e.g., wheezing, cyanosis, nasal flaring).

Tachypnea (Fast Breathing):

 More than 20 BPM in adults


 More than 30 BPM in children

Bradypnea (Slow Breathing):

 Less than 12 BPM in adults


 Less than 18 BPM in children

6. Supporting Patients on Oxygen Therapy

 Ensure oxygen is administered correctly as per prescription.use of nasal prongs , non


rebreather mask,, mechanical ventilation
 Reposition nasal prongs or face masks for comfort and effectiveness.

7. General Precautions for Safe Use of Oxygen

 Keep oxygen equipment clean and properly maintained.


 Avoid open flames, smoking, or flammable materials near oxygen sources.
 Check tubing for kinks or leaks.
 Ensure proper storage of oxygen tanks.
