Intended Learning Outcomes

At the end of this learning activity, the FS students should be able to:
1. identify the school forms, its purpose and uses;
2. download and fill-out sample DepEd school forms; and
3. do sample computation of grades using the DepEd assessment criteria.

Introduction

The K-12 Grading System, Electronic Class Record Templates, and DepEd Forms
The K to 12 Basic Education Program uses standards- and competency-based grading system. When we say grading
system we mean a system that educators use in a particular institution to evaluate each individual student’s performance and
learning. Grading can include letter grades, percentages and even a simple pass/fail. Grades can be attached to physical activities
like writing assignments, lab work, projects, reports and tests. In DepEd, all grades are based on the weighted raw scores of the
learner’s summative assessments. These summative assessment can be classified into three components, namely, written works
(WW), performance task (PT), and quarterly assessment. These three will be the bases for grading. The nature of the learning
area defines the way these three components are assessed.
For Kindergarten, checklist and anecdotal records are used instead of numerical grades. These are based on learning
standards found in the Kindergarten curriculum guide. For grades 1 to 12, numerical grades are used. The minimum grade needed
to pass a specific learner’s area is 60, which is transmuted to 75 in the report card. The lowest rank mark that can appear on the
report card is 60 for quarterly grades and final grades.
Teachers in the Basic Education implementing the K to 12 program uses electronic class record in excel format to
facilitate high level of accuracy in the computation of the learner’s grade. Each learning area has its specific e-class record. Aside
from these, different DepEd forms are accomplished at varied time within a school year.
In this activity, FS 2 students will be exposed to various DepEd forms and e-class records. They will be given
opportunities to examine and accomplish different forms, compute quarterly grades and final grades, and accomplish e-class
records aligned with their specialization.

• Learner C did not get a Very Satisfactory grade but an Outstanding one. Learner C received a quarterly grade of 91,

Briefly describe the approach you used to systematically compute the grade of the learners.

• The approach I used to compute the grades is by following the systematic approach outlined in the DepEd guidelines.
The list of components and their corresponding percentages for high school and senior high school are as follows. After
that, I input the Highest Possible Scores (HPS) for each component into the E-Class Record and then input each
learner’s actual scores. The system automatically calculates the percentage score for each component, applies the
preset weightings, sums the weighted scores to produce the Initial Grade and Quarterly Grade.

My Analysis

Answer the following questions:

1. Which is easier to do, manually compute the grade of the students or the use of e-class record? Why?
Using an e-class record is easier than computing grades manually. It helps save time because it can add and compute grades
automatically. You just need to type the scores, and the computer will do the rest. It also helps avoid mistakes in the
computation. If there are changes, you can edit the scores easily without starting over. Unlike manual computation, where you
need to compute everything by hand, which takes more time and effort. That’s why using an e-class record is better and more
convenient for students and teachers.

2. Is it important to employ a systematic approach in computing the grades of students? Justify your answer.
Yes, it is essential to employ a systematic approach when computing students grades to ensure that the process is organized,
consistent, and transparent. By following a clear and structured method, teachers can accurately track each component of
assessment, such as quizzes, performance tasks, and participation. This reduces the chance of missing any data and
promotes fairness and accountability. A systematic approach also allows educators to clearly explain how a students final
grade was calculated, which builds trust among learners, parents, and colleagues.

3. Why is there a need to accomplish the different school forms?


There is a need to accomplish the different school forms because they serve as official records that help ensure the accuracy,
accountability, and transparency of the school system. These forms document essential information such as student
enrollment, attendance, academic performance, health records, and promotion status. It is important, especially in cases where
students or parents raise concerns or complaints about grades, as having complete and accurate school forms allows teachers
to present concrete evidence to support their records and decisions.

4. When are the different school forms need to be accomplished? Why?


The different school forms must be completed at specific times throughout the academic year, based on their purpose. For
example, School Form 1 (SF 1) - School Register, which is submitted before the school year starts, ensures that the school
has accurate records of all students, allowing for proper class assignments, resource allocation, and updated official records.
Another example is the SF 9 - the Learner Progress Report Card, is completed at the end of each grading period to provide
an official record of students academic performance. Timely submission of these forms is crucial for tracking student progress,
ensuring transparency, and maintaining effective communication between the school, parents, and students.

5. What will happen if the school forms are not accomplished on the specified schedule?
If school forms are not completed on time, it can lead to several problems. There could be delays in reporting and evaluating
students progress because these forms track important information like attendance and grades. This could also delay the
submission of reports to the division or regional office.

My Reflection

Answer the following questions:

1. If you are the teacher, what approach will you use to systematically compute the grades of you students? Why do you
prefer that approach.

If I were the teacher, I would use the DepEd e-Class Record as my approach to systematically compute the grades of my
students. This tool is aligned with DepEd Order No. 8, s. 2015 and automatically calculates grades based on the correct
weight distribution for Written Works, Performance Tasks, and Quarterly Assessment.
I prefer this approach because it ensures accuracy, consistency, and efficiency in computing grades. It minimizes human error,
saves time, and allows me to focus more on evaluating the quality of student work rather than doing repetitive calculations.
The e-Class Record also helps in organizing student data, making it easier to monitor progress and provide timely feedback.

2. Which of the different school forms is the easiest to accomplish? Why?


The easiest school form to accomplish is SF 2 - Learner’s Daily Class Attendance. This form involves recording the daily
attendance of students, which is a routine task that can be done quickly and efficiently. It only requires basic information such
as whether a student is present, absent, or excused, without the need for additional details or complex data entry. Since it is a
daily activity, it becomes part of the regular routine, making it easy to complete. Unlike other forms, which often require more
detailed data collection. SF 2 is simple and straightforward, making it the least time-consuming and complex to accomplish.
3. Which of the different school forms is the most difficult to accomplish? What will you do to lessen the difficulty encountered
in accomplishing this form?
The most difficult forms to accomplish is the SF 4 – Summary Enrollment and Movement of Learners because it requires the
collection and regular updating of detailed information, such as student enrollment, attendance, dropouts, and transfers. Keeping
track of these changes can be time-consuming and complex, especially when dealing with large amounts of data and frequent
updates. To lessen the difficulty in accomplishing the SF 4 – Summary Enrollment and Movement of Learners, what I will do is
have regular data updates. This way, I won't feel overwhelmed, and it will be more manageable to accomplish. Regular updates
ensure that data stays organized, making the task easier to handle and less stressful.
