NB: Experience!!! (Major Contra-indication: Inexperience)/ Vacuum easier to use and causes less
trauma ( soft cups)/ Forceps more effective but require a highly skilled individual (High risk for litigation)
1. Indication
2. HELP: Nursing staff, pediatrician (risk of birth asphyxia/ shoulder dystocia), anaesthetist
3. Discuss contraindications & possible complications
4. Consent
5. Fulfill prerequisites: LEE.I/ Analgesia/ Pelvic exam( cervix fully dilated, membranes ruptured,
position & level of the head NB!!!, exclude CPD
6. Equipment: vacuum vs forceps (discuss pros & cons with pt); test equipment
7. Back-up plan: if it fails → C/S
Vacuum can be performed by midwife at Community Health Center provided no Head palpable above
the brim
