I, Ananya Mehta, daughter of Suresh Mehta, wife of Rohit Verma, aged 28
years, residing at House No. 128, Sector 14, Panchkula, Haryana – 134113, do
hereby solemnly affirm and declare as under:

1. That my birth/maiden name is Ananya Mehta.

2. That after solemnizing my marriage with Rohit Verma, I am now known


as Ananya Rohit Verma for all social and official purposes.

3. That I was married to Rohit Verma on 18th November 2024 at Kalyan


Mandap, Sector 5, Panchkula, as per Marriage Certificate No.
HR/PNK/MR-5521 dated 25th November 2024, issued by the Sub-
Divisional Magistrate, Panchkula.

4. That my Aadhaar Number (XXXX-XXXX-7890) remains unchanged and


valid.

5. That my PAN Card (BCKPM4321Q) also remains the same post-


marriage.

6. That my Passport No. Z2543218, issued at Chandigarh, shall be updated


to reflect my new name.
7. That the name “Ananya Rohit Verma” shall be read and understood as
follows:
First Name: Ananya
Middle Name: Rohit
Surname: Verma

8. That the names “Ananya Mehta” and “Ananya Rohit Verma” refer to the
same individual, i.e., myself.

9. That I am executing this affidavit to declare my change of name after


marriage and to enable updates in all relevant records and documents,
including but not limited to government records (UIDAI, Income Tax,
Passport), educational and professional certificates, and banking and
financial institutions.

I verify that the contents of this affidavit are true and correct to the best of my
knowledge and belief and nothing material has been concealed therefrom.
