ANEMIA

SUBJECTIVE DATA:
A 29 years old female patient was admitted to hospital
with chief complaints of high grade fever, chills, body
pain, headache and generalized weakness.
Past medical history: fever
Past medication history: Dolo ;650gm
ANEMIA:
OBJECTIVE DATA:
Blood pressure: 120/90 mmhg PCV(35-50%): 2.55 gm%
Pulse rate : 78/min platelet (1.5-4.0lac/m^3): 4.51 lac/m3
Temperature : 102’F lymphocytes(20-40%) : 15%
Chloride(95-105meq/l) : 107 meq/l ESR(<20mm 1 st hr): 43 mm
Hb(11-18g/dl) : 8.1 gm/dl SGOT( 5-40U/L): 55 U/L
RBC COUNT(3.8-4.8ml/m^3): 3.87 ml/m3 SGPT(upto 45 U/L ): 64 U/L
FINAL DIAGNOSIS: ANEMIA

ASSESSMENT:
Based on subjective and objective data the patient was finally diagnosed with anemia.
As the patient had chief complaints of high grade fever, body pain, chills headache,
generalized weakness so they had started the drugs as follows according to day wise .
DAY1:
Initially the physicians prescribed with glucose administration.
Cephalosporin antibiotic ceftriaxone and tetracycline antibiotic doxycycline is giving
to the patient had anemia symptoms which may be cause by H. pylori bacteria.
ASSESSMENT:
PPI pantoprazole was given to the patient to reduce drug induced ulcer .
Vitamin C and vitaminB1 supplement was given to the patient to balance vitamins in the
body.
Analgesic and antipyretic Paracetamol was given to control high grade fever , body pain.
Ferritin was also introduce to the patient to supply iron supplement to supply body and to
treat anemia.
DAY 2:
They are following up all the medications as prescribed in day 1 until the patient was
recovered fully .
They had stopped ondansetron as the patient had no symptoms of nausea and vomiting .
PLAN:
GOALS:
To reduce temperature
To reduce pain
To control anemia
To get better healthy life style
PATIENT COUNSELLING:
1. Food which is iron rich should be included in diet such as spinach, green, leafy
vegetables beetroot, carrot , pomegranate etc.
2. Yoga should be done and breathing exercise help in receiving good amount of oxygen.
3. Medication should be taken as prescribed by physicians and should not skip any does if
it does then take in next dose.
4. Avoid junk food, fatty food and food which added more preservatives.
5. Fruits such as orange, paramagnet should be taken.
6.Drink plenty of fluid in everyday upto4-5 liters per day.
7. Take plenty of rest and try to be stress-free.
8.Maintain proper hygienic condition.
