S1 Introduction

This document provides an overview of integrated management of neonatal and childhood illness (IMNCI) strategies in Ethiopia. It discusses that while under-five mortality has decreased in Ethiopia, pneumonia, diarrhea, malaria, measles and newborn issues still cause over two-thirds of child deaths. The IMNCI strategy aims to reduce mortality and morbidity from major childhood illnesses through integrated management of sick children in facilities, health system strengthening, and community promotion. The case management process involves assessing and classifying illnesses as red, yellow or green and providing treatment, counseling and follow-up care. The objectives and advantages of the IMNCI approach are also outlined.

Session 1

Promoting Child Survival, Growth and

Development and Apply IMNCI

For:Generic Level IV Health Extension Students

By: course instructors

Arba Minch Ethiopia, 20223

1
Session Objective
At the end of this session, you will be able to;

 Define and use all of the keywords

 Identify the major causes of under five mortality

 Define the importance and objectives of the IMNCI

strategy

 Summarize the main steps of the IMNCI case

management process

2
Introduction
 Ethiopia has recorded significant reduction in childhood
mortality and had achieved the MDG4 target in 2012
 Several factors are believed to contribute to the reduction
in under five mortality including;
The improvement in overall socio-economic status
and
The significant increase in access to primary health
care services
 However still many children die before their 5th birth day
3
Introduction….

 When we see the mortality trend from 2005 to 2019;

 Under five mortality decreased from 123 in 2005
to 59 deaths per 1,000 live births in 2019
 Neonatal mortality declined modestly from 39 in
2000 to 33 deaths per 1,000 live births in 2019

4
Introduction….

 In Ethiopia, based on the 2014 WHO/CHERG

estimates, over two-thirds of children deaths are
caused by;
 Pneumonia, diarrhea, malaria, measles and
problems of the newborn

 At the background of these causes of death are HIV

and malnutrition 2% and 50% respectively

Components of IMNCI

The IMNCI strategy includes three important

components:
1. Integrated management of ill children in facilities
and health centers
2. Health system strengthening, particularly drugs
and logistics support
3. Community IMNCI, or promotion of key family
and community practices 7
Components of IMNCI…

8
Objectives of IMNCI Strategy

1. To reduce mortality and morbidity associated with

the major causes of disease in children less than five
years of age, and

2. To contribute to the healthy growth and

development of children

9
Advantages of IMNCI
 Accurate identification of illness

 Focuses on care of the child as a whole and not on the

reason for the visit

 Ensures integrated management of all prevalent

illnesses that the child may present

 Ensures the early identification and prompt referral

of all seriously ill children
10
Advantages…

More appropriate & combined treatment

 Promotes rational use of drugs

 Attention to the health needs of the mother

 Strengthening of preventive services

11
IMNCI Case Management Process
 In order to select the appropriate chart and begin the
IMNCI assessment process knowing the age of child is
necessary
 The IMNCI case management process is presented on
two different sets of charts:
 One for managing sick young infants aged from birth
up to 2 months and
 A separate one for managing sick children aged from 2
months up to five years
12
IMNCI Case Management Process…

 A mother may brought her child to a health facility as

an initial visit or as a follow visit;

 Initial visit: If this is the child’ s first visit for this
episode of an illness or problem
 Follow up: If the child was seen a few days before
for the same illness, this is a follow-up visit

13
IMNCI Case Management Process…

IMNCI case management process has the following

steps:
 Assess the young infant or child
 Classify the illness
 Identify treatment
 Treat the young infant or the child
 Counsel the mother
 Follow-up care provision 14
IMNCI Case Management Process…

Classification
 Classify a child’s illnesses using a colour – coded
classification system
 Pink/Red : Urgent pre-referral treatment and
referral
 Yellow: Specific medical treatment and advice
 Green: Simple advice on home management

15
IMNCI Case Management Process…

For each visit, when you see the mother, or the child’s
caregiver, with the sick child:
 Greet the mother appropriately and ask about the
child
 Take the child’s weight, temperature, and
length/height and record the measurements
 Ask the mother what the child’s problems are
 Determine if this is an initial or follow-up visit for
this problem
