How do you identify and prioritize the most critical IT risks for your organization?
Steps 4 and 5 are the most critical piece of an IT risk mitigation effort. It is very difficult come up with priorities balancing all stakeholder needs. Don’t just simply communicate your risk action plans to stakeholders as this step says, instead work with all parties especially business team collaboratively to align your organization's strategic goals and objectives and prioritize risks that pose a direct threat to critical business processes, customer data, intellectual property, or compliance requirements etc. Balancing all these with cost/budget, resource availability etc., is what makes these steps difficult.

How do you measure and improve the effectiveness and maturity of your vulnerability management program?
Could not agree more on the importance of prioritization and this is the one area most organizations don't do well, at least the ones that I was part of in the last decade or so. Often, they tend to do numbers game by improperly using the metrics to mandate priorities that is not well aligned with product priorities and most importantly, loose the focus on realistic and critical issues that impact the organization negatively.

What are the most useful tools and resources for threat and vulnerability management in code review?
While static code analysis tools can identify potential issues early on, improve quality etc, it can flag code that is not problematic or have material impact with large number of issue counts which can lead to wasted time and resources on research and prioritization. Especially if your InfoSec team improperly uses these metrics and mandates engineering to fix all flagged issues can certainly lead to colossal failure. The effective way to make best use of static analysis tools is to have InfoSec and the engineering teams work closely and define the policy for scan so realistic and critical/important issues bubble up to be addressed immediately rather than trying to fix everything.
