About us
ELDAC is a national specialist palliative care and advance care planning advisory service. ELDAC provides information, guidance, and resources to health professionals and aged care workers to support palliative care and advance care planning to improve the care of older Australians. The project is funded by the Australian Government Department of Health and Aged Care and is delivered by a consortium of three universities and four national peak bodies from aged care, palliative care, and primary care. The ELDAC website is free to access and offers a range of information and practical tools to support clinicians and care providers to plan and provide quality palliative and end life care. Various printed resource packs can also be ordered via the ELDAC website free of charge. The ELDAC project also offers curated collection of resources and tools, as Toolkits, to support service level processes around palliative and end of life care. There are also several interactive digital tools designed to help aged care workers such as a Home Care App, a Self-Care Room, and a palliative care Digital Dashboard, to name a few. Aged care services wishing to work even more closely with the project can participate in the ELDAC Service Development Program and have a dedicated ELDAC facilitator help build their service level capability around end of life and palliative care. ELDAC works in partnership with and builds on the work and resources developed by other National Palliative Care projects including, but not limited to, palliAGED, CareSearch, PEPA, PACOP, Advance Care Planning Australia. ELDAC builds end of life and palliative caregiving capability at the care provider, service, and sector level. 💜 𝗜𝘁’𝘀 𝗡𝗮𝘁𝗶𝗼𝗻𝗮𝗹 𝗖𝗮𝗿𝗲𝗿’𝘀 𝗪𝗲𝗲𝗸 — a time to recognise, celebrate, and support the incredible contribution of unpaid carers across Australia. At RePaDD, we know that carers play an essential role in providing physical, emotional, and social support for people who are ageing, living with serious illness, or approaching the end of life. Yet, as demand for unpaid care continues to grow, so too does the need for stronger policy, service, and research support to ensure carers can continue to care — and to thrive. Earlier this year, RePaDD convened a 𝗿𝗲𝘀𝗲𝗮𝗿𝗰𝗵 𝗿𝗼𝘂𝗻𝗱𝘁𝗮𝗯𝗹𝗲 bringing together carers, advocates, policy makers, and researchers to explore one key question: 👉 𝗪𝗵𝗮𝘁 𝗱𝗼 𝘂𝗻𝗽𝗮𝗶𝗱 𝗰𝗮𝗿𝗲𝗿𝘀 𝗻𝗲𝗲𝗱 𝘄𝗵𝗲𝗻 𝗲𝗻𝗱-𝗼𝗳-𝗹𝗶𝗳𝗲 𝗮𝗻𝗱 𝗽𝗮𝗹𝗹𝗶𝗮𝘁𝗶𝘃𝗲 𝗰𝗮𝗿𝗲 𝗮𝗿𝗲 𝗽𝗮𝗿𝘁 𝗼𝗳 𝗰𝗮𝗿𝗶𝗻𝗴? It is an opportunity to honour the contributions of older people to society and highlight the importance of their dignity, independence and inclusion. I am proud to be a member of the ELDAC aged care team and feel privileged to be supporting aged care services nationally who do inspiring things to deliver quality person centred care experiences. 📣 The End of Life Directions for Aged Care (ELDAC) Linkages Program is calling for expressions of interest from aged care providers who wish to strengthen their palliative care and advance care planning processes and capacity. The ELDAC Linkages Program is seeking a diverse range of eligible aged care services (residential and home care package providers) from across Australia to participate in this initiative, funded by the Australian Government Department of Health Disability and Ageing. Start of last day of Ageing Australia 2025 Conference. Seeing a changed sector. One that understands the challenges but has self-belief about its value, and its skills and capacity to care and to support ageing in Australia. Also seeing the acknowledgement of palliative care and end-of-life care as part of care and part of business. Included in the conference agenda, in panels and presentations, and also a source of active discussions at the the National Palliative Care Program booths. Good to hear from Palliative Care Australia, from providers and from clinicians on how to support older people not only as they age but as they come to the end of their lives. So needed, as in 2023–24 over 84,000 people died while receiving aged care services. The strengthened Standards now formalise the need for end-of-life care to be evidence-based and person-centred. Outcome 5.7 makes this explicit, while Outcome 2.9 reminds providers of their responsibility to ensure their staff are equipped to perform their roles. This is part of broader reforms looking at rights-based care and a sector that has systems, processes, and tools to meet these obligations. But most importantly a culture that sees the older person not only at the centre of care but as central to the care purposes and processes. I would encourage all services to connect with palliAGED and ELDAC aged care if you need ideas, support or resources to help you care for older people with palliative care and end of life care needs.
