🌍 Every year, the new #Guidelines of the European Society of Cardiology (#ESC) shape how cardiologists practice medicine. 📖 Today at the #ESCCongress in #Madrid, the 2025 #Guidelines were unveiled — and they mark a #historic #milestone: ➡️ For the first time, all new #ESCGuidelines include dedicated sections on #sex and #gender differences in cardiovascular health! New British Journal of Anaesthesia commentary on qNOX vs haemodynamic surrogates. Why this matters ? I´d like to share our commentary in the BJA on the interpretation of the qNOX index and its alleged inferiority to heart rate and mean arterial pressure. This short piece clarifies how to evaluate qNOX on its own terms rather than as a generic “nociception detector.” In collaboration with Cristian Aragón, we highlight two essentials: - qNOX is more probabilistic than deterministic. It estimates the likelihood of a response under the current anaesthetic/analgesic state. So, stable qNOX during adequate analgesia often means accurate prediction, not “insensitivity.” - Tetanic stimulus ≠ validated nociceptive input (as used). The protocol’s stimulus does not align with validated paradigms in the literature (including those used to validate tools like NOL or pupillometry). In one cited paper there’s no tetanic stimulus; in another, repeated stimuli over short windows likely introduce temporal summation not a fair test of qNOX behaviour. Why this could move the needle? Encourages fit-for-purpose validation of nociception monitors (probabilistic tools should be judged with probabilistic endpoints). Helps avoid methodological artefacts (stimulus choice, averaging windows, sampling rates) that can mislead clinical interpretation. Highlighting integrins from extracellular vesicles as key players in the tumor microenvironment and metastasis: in this review article, Corina Ciobanasu at Alexandru Ioan Cuza University and Christophe Le Clainche at Institut de Biologie Intégrative de la Cellule - I2BC - discussed recent advances on the role of extracellular vesicle integrins in tumor initiation, proliferation, migration, and invasion. Integrins, a diverse family of cell adhesion molecules, were shown to regulate essential processes such as tumor differentiation, migration, growth, and neovascularization. Tumor-derived exosomes, a subtype of EVs, were found to be enriched in integrins reflective of their parent cells 🔗 https://lnkd.in/eurzD8wT These exosomal integrins promoted extracellular matrix remodeling, immune evasion, and vascular changes, and were strongly associated with tumor progression and organ-specific metastasis. Beyond their mechanistic role, exosomal integrins demonstrated potential as diagnostic and prognostic biomarkers due to their tissue-specific expression profiles. They also emerged as attractive therapeutic targets for disrupting tumor–stroma interactions and limiting metastatic dissemination.
