β-hCG (Beta–Human Chorionic Gonadotropin) Overview • β-hCG is a glycoprotein hormone produced mainly by the placental syncytiotrophoblast cells during pregnancy. • Consists of two subunits: • Alpha subunit – similar to LH, FSH, TSH • Beta subunit – unique to hCG, used for specific testing Functions • Maintains corpus luteum in early pregnancy → continues progesterone and estrogen production until the placenta takes over. • Supports implantation and early fetal development. • Stimulates male fetal testes to produce testosterone. Normal Values • Non-pregnant: < 5 mIU/mL • Pregnancy: • Detectable ~6–8 days after fertilization in blood • Peaks at 8–10 weeks, then declines and plateaus Clinical Uses 1. Pregnancy detection • Urine pregnancy test (qualitative) • Serum β-hCG (quantitative) – more sensitive and can detect earlier 2. Monitoring pregnancy progress • Abnormal rises may suggest ectopic pregnancy or miscarriage 3. Diagnosis and monitoring of trophoblastic disease • Hydatidiform mole, choriocarcinoma 4. Tumor marker in some cancers • Testicular germ cell tumors, ovarian germ cell tumors Causes of Elevated β-hCG • Normal pregnancy • Multiple pregnancy (twins, triplets) • Molar pregnancy (hydatidiform mole) • Choriocarcinoma • Germ cell tumors (testis, ovary) • Certain non-trophoblastic cancers (rare) Causes of Low or Abnormal β-hCG in Pregnancy • Ectopic pregnancy • Threatened or incomplete miscarriage • Blighted ovum • Intrauterine fetal demise
