🚨 New publication in collaboration with American College of Radiology (ACR). Susceptibility-weighted imaging (SWI) is often considered to be more sensitive than T2* GRE when it comes to detecting signals associated with microhemorrhages and superficial siderosis – the two factors that determine ARIA-H severity. However, the magnitude of these differences, and their impact on clinical decision-making is unclear. In collaboration with the ACR, we investigated this impact in a recently-published study: “Monitoring of amyloid related imaging abnormalities: SWI vs T2-GRE”* (Sima et al., 2025, The Journal of Prevention of Alzheimer’s Disease). The study, informed by clinical trial data from aducanumab, lecanemab, and donanemab, shows that susceptibility-weighted imaging (SWI) detects amyloid-related imaging abnormalities (ARIA-H) with significantly higher sensitivity than T2-GRE* — resulting in over 50% more moderate-to-severe ARIA cases being identified. This finding has significant implications: sequence choice can influence treatment eligibility, therapy continuation, and alignment with FDA-approved protocols. Since current safety guidelines and drug labels are based exclusively on T2*-GRE, radiologists and clinicians need to understand how imaging protocols shape #ARIA assessment and patient outcomes. 𝐓𝐡𝐞 𝐦𝐨𝐬𝐭 𝐥𝐢𝐟𝐞-𝐬𝐚𝐯𝐢𝐧𝐠 𝐏𝐄𝐓 𝐟𝐢𝐧𝐝𝐢𝐧𝐠 𝐢𝐬𝐧’𝐭 𝐚𝐥𝐰𝐚𝐲𝐬 𝐚 𝐭𝐮𝐦𝐨𝐫; sometimes it’s an 𝐢𝐧𝐟𝐥𝐚𝐦𝐞𝐝 𝐚𝐫𝐭𝐞𝐫𝐲. 🔥 ⸻ In my last post, I shared how FDG-PET sees more than cancer; it also reveals 𝐢𝐧𝐟𝐥𝐚𝐦𝐦𝐚𝐭𝐢𝐨𝐧 𝐚𝐧𝐝 𝐢𝐧𝐟𝐞𝐜𝐭𝐢𝐨𝐧. Nowhere is that more powerful than in cases of 𝐯𝐚𝐬𝐜𝐮𝐥𝐢𝐭𝐢𝐬. ——— Giant cell arteritis (GCA) and large-vessel vasculitis 𝐝𝐨𝐧’𝐭 𝐬𝐭𝐨𝐩 at the temporal artery. They can silently involve the aorta, carotids, or subclavian arteries, often before symptoms are obvious. That’s why PET/CT is no longer just “nice to have”; it’s 𝐠𝐮𝐢𝐝𝐞𝐥𝐢𝐧𝐞-𝐛𝐚𝐜𝐤𝐞𝐝: 📄 𝐀𝐦𝐞𝐫𝐢𝐜𝐚𝐧 𝐂𝐨𝐥𝐥𝐞𝐠𝐞 𝐨𝐟 𝐑𝐡𝐞𝐮𝐦𝐚𝐭𝐨𝐥𝐨𝐠𝐲 /𝐕𝐚𝐬𝐜𝐮𝐥𝐢𝐭𝐢𝐬 𝐅𝐨𝐮𝐧𝐝𝐚𝐭𝐢𝐨𝐧 𝟐𝟎𝟐𝟐: Endorses FDG-PET/CT to detect large-vessel involvement in GCA, alongside ultrasound and MRI. 📄 𝐄𝐔𝐋𝐀𝐑 𝟐𝟎𝟏𝟖: FDG-PET/CT is recommended as a first-line imaging modality in suspected large-vessel vasculitis when available. ⸻ ◾ 𝐖𝐡𝐲 𝐏𝐄𝐓/𝐂𝐓 𝐦𝐚𝐭𝐭𝐞𝐫𝐬 ✅ 𝐖𝐡𝐨𝐥𝐞-𝐛𝐨𝐝𝐲 𝐜𝐨𝐯𝐞𝐫𝐚𝐠𝐞 → maps vascular inflammation that biopsy and ultrasound miss. ✅ 𝐄𝐚𝐫𝐥𝐲 𝐝𝐢𝐚𝐠𝐧𝐨𝐬𝐢𝐬 → sensitivity up to ~90% for large-vessel vasculitis, especially before steroids. ✅ 𝐆𝐮𝐢𝐝𝐢𝐧𝐠 𝐭𝐡𝐞𝐫𝐚𝐩𝐲 → helps tailor immunosuppression and monitor response over time. ⸻ 𝐁𝐞𝐲𝐨𝐧𝐝 𝐭𝐡𝐞 𝐧𝐮𝐦𝐛𝐞𝐫𝐬: 𝐩𝐚𝐭𝐢𝐞𝐧𝐭 𝐞𝐱𝐩𝐞𝐫𝐢𝐞𝐧𝐜𝐞 👩🦰 For patients, this isn’t just about images. 🙏 A clear PET can mean 𝐚𝐯𝐨𝐢𝐝𝐢𝐧𝐠 𝐢𝐧𝐯𝐚𝐬𝐢𝐯𝐞 𝐛𝐢𝐨𝐩𝐬𝐲. 🙏 Detecting hidden arterial disease early can 𝐩𝐫𝐞𝐯𝐞𝐧𝐭 𝐜𝐚𝐭𝐚𝐬𝐭𝐫𝐨𝐩𝐡𝐢𝐜 𝐜𝐨𝐦𝐩𝐥𝐢𝐜𝐚𝐭𝐢𝐨𝐧𝐬 like vision loss or stroke. 🙏 Most importantly, it offers something patients crave: 𝐜𝐞𝐫𝐭𝐚𝐢𝐧𝐭𝐲 𝐢𝐧 𝐚 𝐝𝐢𝐬𝐞𝐚𝐬𝐞 𝐭𝐡𝐚𝐭 𝐨𝐟𝐭𝐞𝐧 𝐡𝐢𝐝𝐞𝐬 behind vague fatigue, headaches, and fear. ⸻ 🔻 Limitations to keep in mind 📍 Uptake fades quickly after 𝐬𝐭𝐞𝐫𝐨𝐢𝐝𝐬 (best if done within 3–5 days of treatment start) 📍 𝐒𝐦𝐚𝐥𝐥-𝐯𝐞𝐬𝐬𝐞𝐥 𝐝𝐢𝐬𝐞𝐚𝐬𝐞 remains invisible. 📍 𝐀𝐭𝐡𝐞𝐫𝐨𝐬𝐜𝐥𝐞𝐫𝐨𝐬𝐢𝐬 𝐚𝐧𝐝 𝐢𝐧𝐟𝐞𝐜𝐭𝐢𝐨𝐧 can mimic vasculitis. ⸻ 📌 𝐁𝐨𝐭𝐭𝐨𝐦 𝐥𝐢𝐧𝐞: FDG-PET/CT is now 𝐨𝐟𝐟𝐢𝐜𝐢𝐚𝐥𝐥𝐲 𝐫𝐞𝐜𝐨𝐠𝐧𝐢𝐳𝐞𝐝 by EULAR and ACR as a diagnostic tool in large-vessel vasculitis. It doesn’t just extend the view beyond the biopsy site; it 𝐜𝐡𝐚𝐧𝐠𝐞𝐬 𝐥𝐢𝐯𝐞𝐬 by accelerating diagnosis, guiding therapy, and 𝐩𝐫𝐞𝐯𝐞𝐧𝐭𝐢𝐧𝐠 𝐢𝐫𝐫𝐞𝐯𝐞𝐫𝐬𝐢𝐛𝐥𝐞 𝐜𝐨𝐦𝐩𝐥𝐢𝐜𝐚𝐭𝐢𝐨𝐧𝐬. ⸻ P.S.
