Building our product in Sweden meant we started by playing on hard mode. Swedish patients can read their own medical notes online through a system called 1177. This single fact changed many core assumptions about how we had to build Tandem Health. In many countries, medical notes act as reminder notes for clinicians. In Sweden, they’re detailed documentation for patients to understand their care. Patients actually read them and complain if they’re inaccurate or incomplete. Swedish medical notes also have 50+ sections instead of the simple & standard 4-section SOAP format used elsewhere. Additionally, every clinic creates its own variations of the 50+ section templates. We had to build extreme modularity to handle this complexity. When we expanded to other countries, our product was over-engineered for their simpler requirements. We had superpowers. Language models also perform worse in Swedish than in English, so we had to solve challenges that other companies never faced. The most common antibiotic in Sweden, Kåvepenin, was never correctly transcribed by the leading models. Specialised medical terms in smaller languages were simply not part of the training data. Sometimes you accidentally choose a harder path, which gives you competitive advantages you never expected.​​​​​​​​​​​​​​​​

David Heinemeier Hansson spoke at DTU today. The session cut through noise and gave real clarity on how to think about work, ambition, and growth. Some powerful takeaways: → Work-life balance is not about perfection in a single day. It is about finding rhythm over time and letting good days outweigh the bad. → Pessimists may sound sharp, but optimists are the ones who build and create. Optimism gives you room to think bigger. → Nobody has all the answers. Even the smartest people make wrong bets. What matters is enjoying the process, learning constantly, and staying open. → Movement fuels everything—physically and mentally. Sleep, stoic wisdom, healthy food, and exercise are not side habits.
