We provide services to the residents of Halton, Knowsley, and Liverpool in Merseyside, with a patient population of over 1 million. The services we provide include: • Out of hours GP service • Community Intermediate Care • Healthcare to Asylum Seekers • In-hours primary care service (region-specific) • Primary care in A&E at Alder Hey and RLUH Collaboration and quality are at the heart of what we do and we work with partners and communities to ensure that we meet the needs of patients and commissioners and achieve positive system change. So how can neighbourhood-based approaches make a real difference — improving access, experience, and outcomes for every community? Join us for this powerful panel discussion, chaired by Dave Horsfield, CEO of Primary Care 24, as we explore how the Neighbourhood Health model can be a driver for greater equity across the system.
