The National Maternity and Perinatal Audit (NMPA) has published a new State of the Nation report, looking at birth trends and outcomes in England, Scotland and Wales during 2023. The NMPA, led by RCOG, is the largest quality improvement programme for maternity and neonatal services in the world, providing a unique picture of birth trends and outcomes in Great Britain. The report reveals significant changing trends in how babies are being born. In 2023, there were fewer vaginal births (both with and without instruments) and a rise in caesarean births compared to previous years. The report also found: • Unplanned caesareans rose to 23.1% in 2023 (from 15.5% in 2018/19). • Planned caesareans rose to 16.4% (from 12.1% in 2018/19). • VBAC (vaginal birth after caesarean) rates declined to 14.2%, down from 24.5% in 2016/17. The RCOG does not promote one form of birth over another. Women should be supported to make an informed decision about how they want to give birth, including a discussion on the risks and benefits of both vaginal and caesarean births. The safety and care of the woman and baby during pregnancy, labour and birth should always be the main focus. Changing trends in mode of birth have important implications on maternity services. The report makes five evidence-based recommendations and calls on national governments and maternity providers to address unwarranted variation and deliver consistent, evidence-based care for women and families across the UK. A relatively new role at our trust that involves coordinating the rehab of patients with major trauma, from their admission through to their discharge and beyond, is proving a huge success. The UK is split up into trauma networks, with both Yeovil and Musgrove Park hospitals part of the Severn network. The major trauma centre for the Severn network is at Southmead Hospital in Bristol, with both our acute hospitals acting as trauma units. Lily Jackson (left) is our major trauma rehabilitation coordinator at Yeovil Hospital, with Lynne Thorpe (right) taking on the role for Musgrove Park Hospital. In the past, patients across England who’ve had complex traumas have said they’re so thankful that their lives have been saved, or their limbs have been fixed, but upon leaving hospital they’re still struggling and not getting back to the daily activities that are so important to them. One of the areas identified was the need for a rehabilitation coordinator, who could personalise patients’ rehab plans, and capture what’s really important to them – and that’s where Lily and Lynne fit in! Earlier this year we held our annual peer review to check how we compare to other trusts nationally, and that identified that we’re meeting even more standards for the way we provide rehabilitation after a major trauma, compared to previous years. Also, recent audits have concluded that we are performing above the national average, and we’re working to continuously improve this.
