Family Service of Chester County, a respected nonprofit human services agency serving Chester County, is seeking a Medical Case Manager to join its HIV/AIDS Prevention and Care Program based in Coatesville.

This is a full-time position (37.5 hours per week), with a salary range of $45,000–$50,000 annually. The benefits package is generous and includes:

Position Summary

 The Medical Case Manager will deliver comprehensive medical case management to individuals living with HIV/AIDS of diverse social, ethnic, and economic backgrounds. Key duties include:

• Assessing client needs
  Facilitating access to medical, social, and supportive services
• Making referrals to appropriate programs
• Maintaining accurate and timely documentation in client records
  
  

Requirements

• Bachelor’s degree in a human services–related field.
• At least 2 years of case management experience.
• Strong communication and computer skills.
• Valid driver’s license and reliable transportation.
• Fluency in Spanish is a plus.
• Prior experience in direct services to people living with HIV/AIDS preferred.
• Knowledge of HIV/AIDS disease and treatments.
• Ability to relate to clients of various backgrounds, orientations, and preferences.
• Strong organizational skills and attention to documentation and reporting.
