I had the honor and privilege at #ASA25 to give the Ellison Pierce Patient Safety Lecture sponsored by American Society of Anesthesiologists® Anesthesia Patient Safety Foundation (APSF). I spoke about the huge patient safety challenge in bridging the gap between Standards of Care, which are frequently based on work as imagined, and the work as actually done, where those standards need to be applied. There are #Implementation and #HumanFactors experts to guide us in ensuring that the implementation of Standards is locally appropriate so as to become a sustained part of all clinical care. Among the tools I discussed were #Nudge, a gentle reminder approach to changing behavior and also #FMEA failure mode and effects analysis, a proactive tool WFSA (World Federation of Societies of Anaesthesiologists) UCSF Department of Anesthesia and Perioperative Care, Karen Gelb Medtronic Acute Care & Monitoring BD GE HealthCare Kristi Larson Angie Lindsey Paul Barach, B.Med.Sci, MD, MPH, Maj (Ret.), AUA Jonathan Chejfec-Ciociano Canadian Anesthesiologists' Society
