Knowledge is power. Neighborhood Health Center Senior Director of Quality Frank Smith and providers Dr. Chantal Ogle and Jackie Ryan, FNP-BC participated in the Azara Healthcare conference earlier this month. They discussed Neighborhood's work to improve health outcomes for patients through efforts to ensure patients are coming in for preventative visits, and pairing patients with a usual provider that they see each time for consistency of care. Great work, team! #sharingknowledge #WelcometoNeighborhood
