  • Proud to share our latest episode of Saved by the Belle: Member Stories that Showcase Our Value and Our Mission. Episode 19: When Early Detection Triggers Life Saving Care During a routine visit, a Belle-Certified Community Health Worker checked the vital signs of an 89 year old member living with hypertension, diabetes, hyperlipidemia, and chronic kidney disease. One reading immediately stood out. Blood pressure of 197/90. Following protocol, the CHW quickly joined a telehealth visit with a Belle provider to assess the situation in real time. During the evaluation, the provider learned the member had run out of her blood pressure medication more than a week earlier. A repeat reading showed her BP had risen further to 230/96. Recognizing the severity of the situation, the Belle Provider advised the member to seek emergency care immediately. The member was transported to the hospital, where her systolic blood pressure measured 191 in the emergency room. She was experiencing dizziness and disorientation. Hospital staff stabilized her blood pressure and safely discharged her with close follow up arranged with her primary care provider the very next day. Because of the CHW’s timely observations and our team's coordinated response, a potentially life threatening event was identified early and addressed quickly. This is the power of foot-forward in home care. Being in the right place at the right time, meeting members where they are, and stepping in before a crisis escalates.     Proud to share our latest episode of Saved by the Belle: Member Stories that Showcase Our Value and Our Mission Episode 18: When Timely Intervention Prevents a Crisis Our team recently visited a 79 year old member living with COPD, diabetes, and hypertension. During her monthly visit, our Belle Certified Community Health Worker (CHW) took her vital signs and noticed two concerning findings: an SpO₂ of 84% and a pulse of 108. Following clinical protocol, the CHW immediately joined our Care Manager in the urgent Doxy room for real time evaluation. During the assessment, we learned the member had home oxygen but avoided using it. Our nurse stepped in to explain that her symptoms of low oxygen levels, rapid heart rate, lethargy, and shortness of breath were all signs of hypoxia. She provided education to the member and family about the dangers of prolonged low oxygen and how quickly it can escalate. With this understanding, the member agreed to put on her prescribed oxygen. Within minutes, her SpO₂ and heart rate returned to normal ranges. Before ending the visit, our Nurse Care Manager reinforced the importance of wearing oxygen as directed and checking her levels frequently to prevent future complications. Because of the CHWs timely observations and our teams coordinated response, this member received the right support at the right moment, helping prevent what could have become a medical emergency.
