    Think about this before switching EMRs: A new vendor might catch your eye with a 10% discount. But what does switching actually cost in practice? (Hint: It’s not the data migration. Most vendors handle that part.) The real cost is everything that happens after: - Rebuilding your note templates - Re-training your entire clinical team - Redesigning workflows that took years to fine-tune Even the best tools require buy-in, and most providers don’t have time to relearn a system midweek. It’s a bit like asking your entire company to switch from Mac to Windows overnight. Technically doable but operationally painful. We’ve seen clinics take a few approaches over the years: 1. All at once – risky, but sometimes necessary 2. Provider by provider – works well for smaller practices 3. Location by location – often best for larger orgs If you’re considering a switch, it’s worth thinking beyond the price tag.

    One of the most powerful levers for PCPs to improve patient outcomes is making the right referral to a specialist. Referrals aren’t just logistical, they’re a clinical decision and a reflection of trust. And often, they’re the turning point in a patient’s care journey. That’s why it’s worth being proactive: - Know which conditions you refer out most often. - Keep a vetted list of specialists who meet your standards. - Ask patients for feedback on their referral experience—wait times, appointment availability, outcomes. A timely, well-matched referral improves continuity of care. A bad one can damage trust.

    “I have an AI scribe. Why do I need to QA my notes?” I hear this question all the time. Scribes (human or AI) are great for offloading the burden of documentation. That’s good for burnout, good for efficiency — but it’s not the full picture. A scribe can capture what was said. But they can’t tell you: 1. If the right questions were asked. 2. If treatment decisions were clinically sound. 3. The observations that weren’t verbalized (like the provider’s interpretation of how cultural considerations play into the patient’s condition.) Scribes improve efficiency, but they don’t ensure the session was run correctly to begin with. This is coming up more often as AI tools become standard.
