#MissionMonday Since joining Cadence, this patient has brought their average glucose level down from 175 to 130. They’re feeling more in control, more supported, and deeply grateful -- and so are we. They told us this week: "You're one of the positive parts of my healthcare." These moments matter. They remind us that great outcomes start with great relationships, and that what happens between visits can make all the difference.

This week Cadence helped found a coalition of leading digital health organizations, the Remote Monitoring Leadership Council, with the aim of scaling access to high-quality, tech-enabled care, and ensuring that the tools already improving patient outcomes and lowering costs are set up to thrive. Why now? → High quality RPM works. It's a proven, low-cost solution that helps more patients meet their blood pressure and glucose goals, and avoid costly hospitalizations. Cadence data shows savings of $1,300 per patient within the first year. → But we need public policy to match technological progress. Medicare reimbursement for RPM continues to decline – not because these programs don’t work, but because they’re tech-based, not labor-based. If we want this care model to scale and succeed, CMS and Congress must act. We talk a lot about what it means to give patients peace of mind, and by that, we mean making sure they feel seen and supported. At the heart of quality healthcare is the reassurance that someone is paying attention, that your care team is there when it matters. This week’s #MissionMonday is a powerful reminder of that. One of our newer patients recently shared that after surgery, they were discharged to a rehab facility and felt deeply anxious about returning home alone. They told us they suffer from chronic anxiety and that the idea of managing recovery without support felt overwhelming. But since enrolling in Cadence, that anxiety has lifted. They know that someone is keeping an eye on their vitals, that help is always a call away and that they’re not alone in navigating their health. This is exactly why we do this work, to ensure patients feel safer, more supported and more confident in their care every day.

Happy National Nurses Week! 👩⚕️👨⚕️ At Cadence, our nurses do more than respond – they anticipate. They build trust with patients, catch early warning signs and act quickly to keep people well and at home. Their proactive care is what makes our model work and what makes patients feel seen, supported and safe. Cadence’s Head of Clinical Operations, Chad Greeno, MBA, FACHE, said it best in a video to our team this morning: “They're listening with empathy, they're coaching with compassion, they're monitoring vitals diligently and showing up with a level of consistency, warmth, clinical excellence that truly changes lives for the better.” To every nurse on our team: thank you. You are essential to the future of healthcare we’re building.

Today’s #MissionMonday is a powerful example of how strong communication and collaboration between providers can prevent symptoms from worsening and keep patients out of the hospital: Recently, a heart failure patient’s primary care physician noticed an elevated BNP level – an early warning sign of fluid retention. Within hours, a Cadence RN connected with the patient, ensured they resumed their diuretics, and followed up with their daughter to adjust medications. Since then, the patient’s weight has dropped by 4 pounds, reducing the risk of exacerbation and helping them remain safely at home. Thanks to the coordinated effort between our partner’s PCP, the Cadence clinical team, and the patient’s family, we turned a warning sign into a win for proactive, team-based care.

Cadence is proud to support the Rural Patient Monitoring Access Act, introduced yesterday by Senator Marsha Blackburn (R-TN) and Senator Mark R Warner (D-VA), along with Representative David Kustoff (R-TN), Representative Troy Balderson (R-OH), Donald Davis (D-NC), and Mark Pocan (D-WI). The RPM Access Act would make it easier for seniors in rural and underserved communities – those who face some of the highest rates of chronic disease and greatest barriers in accessing care – to receive high-quality remote monitoring services. This legislation is responsive to Cadence CEO Chris Altchek’s testimony before the House Ways & Means Committee last spring, and is an important first step toward addressing a flawed Medicare reimbursement methodology that pays rural providers less for delivering RPM, penalizing communities that need this care most. The bill has the potential to strengthen access in rural and underserved areas where staggering declines in RPM reimbursement (up to 30%+) have had the greatest impact. Endorsers include leading organizations such as the National Rural Health Association, American Association of Nurse Practitioners, HIMSS, the American Telemedicine Association, the Alliance for Connected Care, Ascension, and Lifepoint Health®. We are happy to see growing validation of the work Cadence is doing to bring proactive care to every senior managing heart failure, hypertension, and diabetes, in rural areas and beyond. We remain committed to sharing and publishing on the impact of high-quality Remote Patient Care, which we know improves clinical outcomes while lowering costs through avoided hospitalizations. We encourage others across the health care and public policy landscape to join us in supporting the RPM Access Act. https://lnkd.in/ep65ueed

#MissionMonday At Cadence, we’re building a system of care that helps America’s seniors stay healthier and at home. One patient told us recently: "I just love hearing from you folks. Every little change you catch and you call me. It’s so nice to know I have someone checking in with me. I just want to thank you all for what you do there." Our Care Team works every day to earn our patients’ trust by catching the small things and acting quickly when it matters most. That’s always priority number one.
