Committed to leading the way We’re Washington’s first not-for-profit to serve Apple Health members. In 1992, Washington’s low-cost clinics (known as Community Health Centers) realized too many people were being neglected by traditional insurance. They got together and started Community Health Plan of Washington. CHPW is the state’s first not-for-profit managed care plan founded by Community Health Centers. Today, we’re still the only not-for-profit to provide managed care to Apple Health members. Always working to improve our services, we expanded to cover Medicare patients in 2007. As we reflect on 2025, the milestones CHPW achieved stand out as powerful examples of our team's commitment to innovative initiatives that improve the health of our members and communities across Washington. One of these milestones was the Foundation for Health Care Quality and the Bree Collaborative presenting Community Health Plan of Washington with the 2025 Mountain Climber Award for Excellence in Health Equity for CHPW’s equity-driven approach to improving diabetes care. The Mountain Climber award is given for exemplary implementation efforts that focus on the Bree pillars of transformation. A multidisciplinary team from CHPW’s Quality, Care Management, and regional care teams implemented innovative strategies like at-home A1c testing, regional community outreach, and Community Health Center partnerships to address disparities across geography, race/ethnicity, and language. “We’re honored to receive the Bree Collaborative Mountain Climber Award for Diabetes Care. This award recognizes our team’s dedication to supporting our members with culturally appropriate education and dedicated resources, affirming that we’re meeting them where they are with high-quality, equitable care,” said Edie Shreckengast, Health Coach in Care Management, who is part of the Diabetes Care Program, and accepted the award on behalf of the team. Building on the momentum from receiving the Foundation for Health Care Quality and the Bree Collaborative’s Trailblazer Award for our Diabetes Care Program in 2024, this year’s Mountain Climber Award further affirms CHPW’s unwavering commitment to improving health outcomes for all Washingtonians through innovative, whole-person care. The Anacortes Family Center is making a lasting impact in the community, and we're proud to support their work as a 2025 Community Investment Fund recipient. The nonprofit was recently featured in the Anacortes American, highlighting its plans to utilize their $10,000 grant for emergency shelter services. This initiative aims to ensure families experiencing homelessness have access to safe, stable housing and critical support. The Anacortes Family Center supports individuals experiencing homelessness on their path to self-sufficiency through emergency shelter, case management, and educational programs. With a success rate of 75–90%, most clients leave the program able to support themselves. This outcome is one of the highest success rates in Washington State.
