I kept seeing articles saying “cannabis doubles heart risk.” But when I started sketching and learning more, I realized: The REAL story wasn’t about cannabis at all. Italy Approves the First Law Recognizing Obesity as a Chronic Disease
🇮🇹 Historic Recognition for Italy The final approval in the Senate of a new law on obesity marks a historic turning point for Italy: for the first time, obesity is officially recognised as a chronic, progressive, relapsing disease, fully included among conditions requiring prevention, early diagnosis, and integrated care within the Italian National Health Service. Historically, obesity, despite being one of the most widespread and impactful health conditions, was often perceived as an “individual problem,” linked mainly to personal choices and responsibility. With this law, however, the Italian Parliament has made a symbolic and substantial step forward: obesity is no longer characterised as just a matter of lifestyle, but is acknowledged as a disease that requires clinical and social care. The law, strongly promoted by Hon. Roberto Pella, President of the Inter-Parliamentary Group on Obesity, Diabetes, and Non-Communicable Diseases (NCDs), reflects a broad political consensus on the urgency of addressing obesity. The Zolotov et al. consensus definitely represents progress. But we also need to talk about what's missing. While celebrating this JAMA Network Open milestone on medical cannabis competencies, we must address a fundamental flaw: the endocannabinoid system gets relegated to a brief introduction before diving into cannabis pharmacology and policy. This backwards approach perpetuates a critical educational blind spot. The ECS isn't cannabis context. It's a master homeostatic network governing stress, pain, immunity, and metabolic homeostasis. It does this together with our gut-microbiome and its signaling is directly influenced by our diets-, exercise patterns, in-real-life social interactions, mindfulness, and integrative holistic practices like acupuncture and yoga. CB1 receptors are among the brain's most abundant, yet most medical students graduate without understanding retrograde lipid signaling or arachidonic acid homeostasis. Our dietary preferences create microbiomes that can either facilitate ECS balance or actively destroy ECS homeostasis. We're teaching cannabis applications before biological foundations. It's like teaching cardiac medications without cardiovascular physiology. The nomenclature problem runs deeper than education gaps. "Endocannabinoid system" suggests these receptors exist for cannabis, when they evolved hundreds of millions of years before the plant developed psychoactive compounds. We've accidentally named a fundamental homeostatic network after one of its exogenous ligands. Imagine calling the cardiovascular system the "digitalis system" because foxglove affects it. The path forward: ✓ Teach ECS physiology FIRST, applications second ✓ Frame it as retrograde lipid signaling—core biology ✓ Stop treating fundamental homeostasis as drug context The Zolotov competencies are necessary. But until we elevate the ECS to its rightful place alongside nervous and endocrine systems, we're building cannabis literacy on a foundation of physiological illiteracy. Medical education deserves better. What does it signal that the President released a pro-cannabis video advocating for hemp derived CBD for seniors. Is it a sign of what's to come? Is the medical community ready to provide the guidance necessary for people to use cannabis and cannabis-derived products safely? We think not. What do you think?
