Understanding CKD risk through the lens of obesity treatment. In a large population-based cohort study in Denmark, researchers followed over 18,000 individuals who underwent gastric bypass or sleeve gastrectomy, comparing them to a matched control group with obesity who didn’t receive surgery. The study concluded that patients who had surgery showed significantly lower long-term risk of CKD and kidney failure. They also experienced higher short-term risks of acute kidney injury and kidney stones. How should these findings guide treatment? Our Chief Medical Officer, Timothy Pflederer, offers his take: "Given the risks of bariatric surgery, a much better approach today is to utilize GLP-1 agonist medications to treat obesity as first line treatment. These medications are effective in weight loss, do not carry risk of early AKI, and have direct kidney and cardiovascular protective benefit. Bariatric surgery should be reserved for patients who are unable to tolerate or not candidates for medical treatment of obesity.
