About us
Grand River Health includes our main hospital campus and our long term care center in Rifle, Colorado. We are also proud to offer services at our satellite clinic in Battlement Mesa, Colorado. The Western Slope of Colorado offers year round recreation with everything the outdoor enthusiast can imagine- all in a picturesque location on the Western Slope of Colorado. Big game hunting, fishing, golf, water sports, world-renowned rock and ice climbing, hiking, biking, snowmobiling and 4-wheeling: It’s all here for you. For those who enjoy the ski slopes- take in the world-class mountains of the Vail and Aspen valleys; just an hour East. When it comes to caring for the well-being of our community, we at Grand River Health are always striving to improve as the lifelong partner in health for the residents of Rifle and its surrounding communities. We are very pleased to have received a 5-Star rating for our care center and a 4-Star rating for the hospital from Centers for Medicaid and Medicare (CMS). This keeps Grand River Health in the top 21% of hospitals in the nation and in the top 5% of long term care centers in the nation. In the spirit of continuous improvement we pursue a 5-Star rating for the hospital because we believe our community deserves only the best in their healthcare. Grand River Health holds a strong belief that community health begins long before you ever see a physician. It is in what you eat, how you recreate, what resources are available to the community and how we, as a healthcare organization, support our community.

Grand River Health is very excited to share a new 5 star rating from CMS for quietness. The five-star rating was based on how quiet patients said their rooms were at night. Patient sleep is vital for healing and recovery.
