I'm delighted to share the first MN EHR Consortium paper focused on our Health Trends Across Communities project published in JGIM Journal of General Internal Medicine! Our paper analyzes statewide EHR data across Minnesota to estimate disease prevalence among people with recent homelessness or incarceration. To our knowledge, this is the first time EHR data have been used to analyze disease prevalence among these populations in the US. We believe this is a scalable model for identifying health needs among vulnerable communities.
