PACE (Program of All Inclusive Care for the Elderly) is an alternative to nursing home care that - through an interdisciplinary team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, transportation coordinators and providers and aides - coordinates and provides preventive, primary, acute, and long-term care services, so older individuals and individuals with disabilities can continue living in the community.
MassPACE is a 501c(6) association whose mission is, through advocacy, education, and policy research, to provide leadership and support for growth, access, quality, and success of PACE in the Commonwealth of Massachusetts.
