Social determinants of health have a significant impact on a person’s well-being and quality of life, and they play a crucial role in a person’s overall health, particularly as they age. If left unresolved, these social barriers to health (i.e., lack of transportation, access to food, financial insecurity, gaps in education, and inadequate support systems) can lead to negative patient outcomes.
