About us

On Lok is a nonprofit organization that empowers older adults to age with dignity and independence. With over 50 years of experience, On Lok is a trusted resource in San Francisco, Santa Clara County, and the Tri-City area of Alameda County. Our mission: to relentlessly pursue quality of life and quality of care for older adults and their families. On Lok founded and still operates the Program of All-Inclusive Care for the Elderly (PACE), which allows eligible seniors to live at home for as long as possible by providing comprehensive medical care and social services. On Lok leverages its unique expertise as the pioneers of PACE through On Lok PACEpartners, a consulting service that supports organizations interested in exploring PACE, developing a PACE program, or improving their PACE outcomes. On Lok also operates On Lok 30th Street Senior Center, the largest multipurpose senior center in San Francisco, which provides a second home, health programs, activities, and case management services to active seniors. While the On Lok Mission Nutrition Program, provides more than 250,000 meals annually to seniors through its dining locations and meal delivery program. In 2021, Openhouse and On Lok joined forces to create Openhouse + On Lok Community Day Services, a safe and life-affirming space for LGTBQ+ seniors to maintain wellness and empower independence.

    The energy in New York City has been inspiring at the 2025 National PACE® Association Annual Conference! Over the past two days, the On Lok team has been actively engaged across sessions, panels, and discussions focused on advancing person-centered care and strengthening the PACE model nationwide. Sessions from the last few days included: Grace Li, Sandy Rodgers, and Lori Frank presented “Philanthropy in the PACE Landscape.” Dr. Jessica Eng and Nathan McKenzie, alongside Dr. Jessica Neil from IOA, presented “Smart Care, Sustainable Costs: Proactive Strategies to Reduce High-Cost Service Utilization in PACE.” Nicole Torres and Agnita Pal presented “Harnessing Data for Health Equity: On Lok PACE’s Journey.” We were also so incredibly proud to see Portia Ravanera, RN Lead, recognized as a 2025 Nurse of the Year nominee for her impact on our community!   • What a way to kick off the 2025 National PACE® Association Annual Conference in New York City! Day 1 brought more than 1,300 attendees to this year’s conference, (with one-third joining for the first time!) On Lok PACEpartners sponsored yesterday’s opening session, featuring keynote speaker Dr. Aditi Nerurkar. We also led a full day of PACE Basics sessions with Nathan McKenzie, Lori Frank, Sandy Rodgers, and Joe Ayer from Dignity Health Connected Living, exploring topics like Developing a High-Functioning IDT and Assessing Organizational Readiness for PACE.     Thank you, Supervisor Danny Sauter, for visiting the On Lok PACE Gee Center! This Center is unique in many ways, and one is that we have two teams – one that is conducted entirely in Cantonese and the other that is multi-lingual. This linguistically and culturally sensitive programming further supports our seniors to live independently in the community. Our staff and seniors were impressed by Supervisor Sauter providing a few minutes of remarks in Cantonese! During the tour we showcased the variety of On Lok PACE services available onsite – from adult day health care and rehab, to our commercial kitchen and clinic - and the Supervisor had a chance to meet many of our staff, some of whom have been with On Lok for more than 20 years. On Lok has multiple centers in District 3, and we look forward to working with Supervisor Sauter in the years to come and elevating the needs of seniors in our community. #aging #sanfrancisco #onlok #seniorcare

    On Lok’s presence is growing in Union City! We recently hosted a tour for Union City Councilmembers Jaime Patino and Jeff Wang to see the progress on the construction of one of our newest locations which is expected to open in early 2026. This 26,000 square foot space will allow On Lok to expand and deepen our reach to continue serving frail seniors in the Tri-City Area through a beautiful, state-of-the-art building.
