Enabling value-based, post-acute care. As post-acute care strives for better health and financial outcomes benefiting patients and plans, providers must have the capability to implement comprehensive care. Our model creates true alignment between the health plan, delegated at-risk entity, clinicians in the field and members, leading to better care, higher satisfaction and lower costs. Our management model enables a broad range of post-acute care options including home healthcare, DME, home infusion, SNF and gap-closure services through our robust value-based model. We go beyond the typical utilization and network management models, leveraging high-performing providers to help ensure patients receive the right amount of care in the most appropriate setting. How we deliver care inside and outside the home is as vital to our model as what we provide. Terrific insights from on the current state of the post-acute care industry.
With the proposed home health payment rule fresh on the minds of providers, I was reflecting on a conversation I had with other industry leaders on the topic of post-acute care at #AHIP2025 conference a few weeks ago. High-performing networks are about enabling providers to deliver high-quality care by reimbursing for value. At #OneHome, we are in a unique position to understand the cost pressures on both health plans and providers.
