About us

Transition Projects provides individuals with the services, resources and tools they need to end their homelessness, secure housing, and maintain that housing. - Each year, we assist more than 10,000 people experiencing homelessness. - On any given day, we help meet the basic needs of more than 700 people through our Resource Center. - On any given night, we provide a safe place to sleep for 700 people with nowhere else to turn. - In any given year, we help place more than 900 people into affordable housing - and then support them in retaining that housing. Whenever and wherever we can, we advocate for policies and initiatives that affirm the right of all people to a place they can call home. Want to join our team of passionate changemakers?   • Transition Projects is excited to partner with the Multnomah County Health Department Mobile Clinic at our Willamette Center shelter! Many of our participants need wellness support when they are in shelter and one of our biggest challenges is getting people to appointments — which are often scheduled weeks into the future and involve coordinating transportation. The Mobile Clinic provides medical, dental, and lab services; and is able to set up ongoing and specialty care. The first day they were on site nine of our participants received medical care, right across the street! All of the participants were pleased with the care they received. Going forward the Mobile Clinic will improve our ability to provide timely, top quality care for our participants — often the first medical care they've had after living unsheltered.   • 🙏 We’re thrilled to once again participate in Willamette Week's Give!Guide! Mark your calendars from November 1 to December 31 to support Portland’s local nonprofits.     A Look Inside Our Resource Center — At Transition Projects, we believe everyone deserves to have a safe place to sleep, nutritious food, and resources to support their journey to permanent housing. For many people in our community experiencing homelessness, their first step starts in Transition Projects' Resource Center. We connect individuals, couples, and families to supportive services and programs 365 days a year. For any immediate needs, we offer day services like showers, laundry, mail, medical, and much more. Visit our Resource Center Monday through Friday from 8:30 a.m. to 7:00 p.m. or Saturday and Sunday from 8:30 a.m. to 3:00 p.m. Know someone in need?   • A Look Back on Our Accomplishments: July 1, 2024 - June, 30 2025 Last year, Transition Projects placed 810 people into affordable housing — including 380 Veterans! Helping people secure — and retain — permanent housing is the ultimate goal of our work. There is still plenty of work to do, but it is clear that when we have enough resources, we can increase the number of people transitioning from shelters into housing. Our work continues to be as urgent and vital as ever, and your support empowers us to serve those who need us the most. We help them find and keep safe, affordable housing. And, providing meals for our shelter guests is our greatest volunteer need. Last year, our wonderful volunteers provided 89,710 meals — totaling 30,632 volunteer hours cooking and serving meals!   • A Look Back on Our Accomplishments: July 1, 2024 - June, 30 2025 Transition Projects knows we can't do it all alone — no agency can. That’s why our community partnerships are so important. Partnerships let us focus on what we’re best at and make sure people experiencing homelessness get well-rounded care — leading to better outcomes. Last year, 26 community partners provided on-site services including medical, dental, legal, ID assistance, haircuts, mental and behavioral health, barrier reduction, and more.
