A Look Inside Our Resource Center — At Transition Projects, we believe everyone deserves to have a safe place to sleep, nutritious food, and resources to support their journey to permanent housing. For many people in our community experiencing homelessness, their first step starts in Transition Projects' Resource Center. We connect individuals, couples, and families to supportive services and programs 365 days a year. For any immediate needs, we offer day services like showers, laundry, mail, medical, and much more. Visit our Resource Center Monday through Friday from 8:30 a.m. to 7:00 p.m. or Saturday and Sunday from 8:30 a.m. to 3:00 p.m. Know someone in need? About us
Transition Projects provides individuals with the services, resources and tools they need to end their homelessness, secure housing, and maintain that housing. - Each year, we assist more than 10,000 people experiencing homelessness. - On any given day, we help meet the basic needs of more than 700 people through our Resource Center. - On any given night, we provide a safe place to sleep for 700 people with nowhere else to turn. - In any given year, we help place more than 900 people into affordable housing - and then support them in retaining that housing. A Look Back on Our Accomplishments: July 1, 2024 - June, 30 2025 Last year, Transition Projects placed 810 people into affordable housing — including 380 Veterans! Helping people secure — and retain — permanent housing is the ultimate goal of our work. There is still plenty of work to do, but it is clear that when we have enough resources, we can increase the number of people transitioning from shelters into housing. Our work continues to be as urgent and vital as ever, and your support empowers us to serve those who need us the most. A Look Back on Our Accomplishments: July 1, 2024 - June, 30 2025 Transition Projects knows we can't do it all alone — no agency can. That’s why our community partnerships are so important. Partnerships let us focus on what we’re best at and make sure people experiencing homelessness get well-rounded care — leading to better outcomes. Last year, 26 community partners provided on-site services including medical, dental, legal, ID assistance, haircuts, mental and behavioral health, barrier reduction, and more. A Look Back on Our Accomplishments: July 1, 2024 - June, 30 2025 In Multnomah County 1 in 2 people experiencing homelessness are living unsheltered. Living on the streets is tough – mentally, physically, and emotionally. Folks experiencing homelessness need access to basic services and safe spaces to be – both during the day and at night. Last year, Transition Projects provided 3,354 individuals a safe bed inside at one of our eight shelters — of that, 72% of shelter guests engaged with our health and wellness team. Recently, Transition Projects' Rent Well team hosted a table at Multifamily NW's Spectrum Convention — celebrating the largest number of attendees to date. One property manager praised Rent Well and shared "13 years ago, they were at the peak of their addiction and had gone through the Rent Well program themselves. From there, they found housing, and once in stable housing, they started working for the property manager of the unit. Eventually, they became the on-site property manager and now oversee six different properties for that company. They love accepting other Rent Well graduates into new rentals!"
