After 10 years working on VBC contracts—most of which depend entirely on successful practice-level transformation—it’s still remarkable how little time is spent learning about the practices we’re seeking to partner with. We all say things like “meet practices where they are,” but in reality, many groups deploy the same one-size-fits-all strategy: 1️⃣Promise practices a big $$ upside at some point in the future 2️⃣Deliver an intimidatingly long patient to-do list 3️⃣Pepper them with performance scorecards that highlight deficiencies It’s hard to see how that builds a meaningful relationship or sustainable change. To a practice doing everything it can to care for patients and stay afloat, receiving another scorecard or vague promise of savings can feel tone-deaf. If you want a practice to change, you have to understand how it works today. At Kairon Health, we’ve developed a strategic planning module for practices with a foot in both FFS and VBC models called Kairon Bridge. It draws on our deep experience with practices that have successfully combined FFS and VBC without burning out their teams, covering 6 domains: Patient Panel Composition – A practice with >50% of lives in risk-based arrangements can often cut daily visits by 25% while growing revenue. With <20%? VBC may be economically unjustifiable. Service Mix – Practices often generate substantial revenue from imaging, procedures, or rounding—but these can carry high overhead. $50K in new VBC revenue might not resonate, but $40K in margin will. Provider Productivity – A provider seeing 12/day needs different support than one seeing 28/day. RCM Performance – Practices leaking 20%+ in collections can’t prioritize VBC until FFS is stabilized. Working Capital – Shared savings are easy to model, but who can wait 18 months to get paid? Staffing & Wage Pressure – Transformation plans fall apart in markets with high labor costs and turnover. Instead of handing a scorecard to a practice and saying “do better,” we help our partners model opportunities over a 5-year horizon, answering questions like: ▪️What happens to our profitability if we see fewer (highly reimbursed) commercial patients to make room for Medicare preventive care? ▪️How long will it take to breakeven on a new clinical hire, and how much cash is needed? ▪️Which revenue streams may conflict with VBC goals and need to be carved out? We don’t assume VBC is right for every practice, or that all are ready to change the same way. We provide a way to tailor strategy—not just by contract, but by site. And when MSOs and ACOs offer scenario-based planning rooted in operational reality, practices engage. Not because they’re told to—but because the future actually becomes navigable.
