In 2009, SMG began to investigate the opportunities and challenges related to gaining PCMH recognition for all 50 of our sites. Opportunities included aligning the overarching organizational goal of value-based care transformation with specific short-term steps (formal NCQA PCMH recognition), taking advantage of new revenues and infrastructure support from commercial payers related to achieving recognition, and proactively brand SMG as a leader in PCMH in advance of the rest of the local and regional health care community. Our challenges centered on the relatively decentralized delivery process at our sites, lack of knowledge of the NCQA recognition process, and buy-in from physicians/sites.
SMG received NCQA PCMH certification for its 4 pilot sites within 6 months of program implementation. By the end of 2010, all 50 SMG sites received NCQA recognition. As a result, the organization became the first Medical Group to receive PCMH recognition in Tennessee and one of the five largest PCMH organizations (by number of sites recognized by NCQA) in the country. SMG has been able to translate NCQA recognition into millions of dollars in additional revenue and infrastructure support from multiple commercial payers, successfully branding itself as a national leader in care delivery, improvements in clinical quality, and multiple short-term “wins” in our ongoing cultural transformation to a value-based organization.
