HomeWell Care Services of Pittsburgh is proud to introduce a new program designed to support families caring for loved ones living with dementia. This initiative, launched by the Centers for Medicare & Medicaid Services (CMS), aims to enhance the quality of life for individuals with dementia, reduce the burden on caregivers, and improve overall family well-being. Additional goals include slowing disease progression, promoting functional independence for as long as possible, and reducing unnecessary healthcare utilization and associated costs. As health care providers and systems work to improve #PatientExperience, our new poll examines an overlooked issue: How often older patients bring "care companions" such as spouses, partners and adult children to their appointments, and what impact this can have. The findings suggest that providers may want to encourage this practice, even for #telehealth, and not just for patients with regular #caregivers. Clinics may even want to consider having staff available to act as care companions for patients with complex needs who don't have someone nearby who can come with them. Dementia affects an estimated 7.2 million older Americans, with most preferring to age at home. Home health care—covering skilled nursing, therapy, and medical social services—has become vital, especially since people with dementia are twice as likely to use it compared to those without. However, challenges persist: Medicare does not cover non-medical home care, Medicaid varies by state, and many families pay out-of-pocket, often facing crippling costs. Care for dementia patients tends to last longer and involve more services, yet payment models like PDGM may limit visits. Families bear significant emotional and financial burdens, and underused respite care adds strain. Policy changes are needed to ensure sustainable, equitable support for dementia care at home. 🔍 Did you know that in South Australia, many hospital beds are occupied by patients who are medically cleared but cannot be discharged - often because there is no aged care or disability placement available? Recent reporting shows 59 patients have each spent over 100 days in SA hospitals waiting for placement - that’s >10,900 bed days collectively. This phenomenon is called “stranded patients” and is putting enormous strain on hospital capacity. Nationally, as many as 1 in 10 public hospital beds are occupied by such patients. What can be done? • More transition / step-down care beds in SA • Better coordination between hospital, aged care, disability sectors • Private providers stepping in to fill gaps At TwinView Health, we’re exploring partnerships to provide some of those transitional supports in regional SA - helping patients move home or into appropriate care, rather than languishing in hospital.
