We can’t lie… 💁🏼♀️ We’re pretty ecstatic about the Street Connect team being named as a finalist in this year’s Youth Affairs Council of Western Australia (YACWA) WA Youth Awards alongside the Vinnies WA Passages Youth Engagement Hub for our role in the Homeless Health Care Youth Project! If you’re wondering why we were nominated, here’s the project breakdown: 👉 It’s all about providing a unique approach to meeting young people’s health needs, breaking down barriers that young people experiencing homelessness have in accessing health and medical support 👉🏽 A Primary Health Nurse and Support Worker attend the Street Connect Bus on a weekly basis and provide wrap around support for case managed young people 👉🏼 It is also the first of its kind in Western Australia! An extra special shout-out goes to legends Will Stapleton, Jasmine Siew, and Sarah Sinclair for all of your brilliant efforts.
