As I continue to get immersed in Oak Street Health, I've been inspired by the work we are doing to redefine primary care for older adults through a value-based care model that integrates physical, behavioral and social care. This comprehensive approach ensures that every aspect of our patients' health is supported, which is crucial for the Medicare population we serve.   Our care model allows providers to spend more time with each patient, building deep, trusting relationships. This connection is essential for managing chronic conditions and enhancing overall patient well-being. By prioritizing longer, quality interactions, we ensure that patients receive the attentive and personalized care they deserve.   We are proud to see the positive outcomes and reduced care gaps, including a 44% reduction in hospital admissions compared to Medicare benchmarks, as a result of our value-based care approach.
