🚨 One of the #commonerrors in surgical practice: treating a skin abscess as if it needs prophylactic antibiotics. ❓ Do you think an adult patient admitted with a #skinabscess who is going for incision and drainage actually needs prophylaxis? And if antibiotics are indicated, when is the most critical time to start them? 💡 Key Point: A skin abscess is a true infection, not a clean surgical wound—so antibiotics here are treatment, not prophylaxis. 🔬 Best Practice for Timing & Use ✅ Obtain cultures first and withhold antibiotics until after drainage/cultures to preserve accuracy. 💉 Initiate #antibiotics before #incision_and_drainage #onlywhen: • Prompt drainage cannot be performed • The patient needs parenteral therapy for severe or rapidly progressive illness (e.g., sepsis) • The patient has risk factors for infective endocarditis (prosthetic valve, prior endocarditis, certain congenital defects, transplanted heart with valve dysfunction) 📌 #Take-home: Avoid unnecessary “#prophylaxis.” Use antibiotics strategically and at the right time to improve outcomes and support antimicrobial stewardship. 📚 Need guidance on implementing Antimicrobial Stewardship (AMS)? Breastfeeding until at least six months helps babies to fight off infections and reduces chronic inflammation, according to a new study. And better understanding the way specific nutrients in breast milk impact the immune system will improve health outcomes for all infants including those not breastfed. The study, led by Murdoch Children’s Research Institute (MCRI) and the Baker Heart and Diabetes Institute (Baker Institute), discovered more clues as to why infants who were breastfed to at least six months of age had fewer infections and less chronic inflammation. Preventing these infections could reduce the rates of many childhood conditions, such as allergies, diabetes and asthma. Published in BMC Medicine, the researchers identified several types of lipids (essential nutrients) in blood samples from breastfed babies that help reduce inflammation, which may reflect the unique nutritional composition of breastmilk. 🚑💊Antibiotic-sparing treatment (ASPT) strategies, such as delayed prescribing and symptomatic treatment, are increasingly recognized as promising approaches to reduce antimicrobial consumption (AMC) in women with uncomplicated urinary tract infections (uUTIs). A new scoping review by Andreas Plate et al. Newcastle University leads UK trial to redefine sepsis treatment. A major UK-wide study, led by Newcastle University, is investigating whether a shorter course of antibiotics could be just as safe and effective for sepsis treatment as the current standard. The research, known as the SHORTER trial, is testing if a five-day antibiotic regimen is as effective as the usual seven-day course. 🟣 𝐀𝐂𝐔𝐓𝐄 𝐎𝐓𝐈𝐓𝐈𝐒 𝐌𝐄𝐃𝐈𝐀 🟣 Acute Otitis Media (AOM) is a sudden infection of the middle ear, characterized by inflammation and fluid accumulation behind the eardrum. It is one of the most common pediatric infections, particularly affecting children between 6 months and 3 years of age. ▪️ Causes AOM is usually caused by bacterial or viral infections that follow an upper respiratory infection: 1️⃣ Bacteria: Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis 2️⃣ Viruses: Respiratory syncytial virus (RSV), influenza, adenovirus Symptoms ✔️ Ear Pain (otalgia) – often worse when lying down ✔️ Fever – may range from mild to high ✔️ Irritability or fussiness – especially in infants ✔️ Hearing difficulty – due to fluid in the middle ear ✔️ Otorrhea – fluid or pus draining if the eardrum ruptures ✔️ Other signs: Poor appetite, vomiting, diarrhea, or sleep disturbances ▪️ Diagnosis ✅ Otoscopy: Shows a bulging, red eardrum with reduced mobility. ✅ Pneumatic otoscopy or tympanometry: Confirms fluid in the middle ear. ✅ Clinical history: Recent upper respiratory infection, sudden onset of pain, and fever. ▪️ Treatment ✔️ Observation (Watchful Waiting): For children older than 2 years with mild symptoms, sometimes AOM resolves spontaneously. ✔️ Antibiotics: Amoxicillin is the first-line treatment for bacterial AOM. Duration: Typically 5–10 days, depending on age and severity. ✔️ Pain Management: Acetaminophen or ibuprofen to relieve pain and fever. ✔️ Surgical Intervention (for recurrent cases): Myringotomy with tympanostomy tubes may be considered for children with repeated infections or persistent fluid. ▪️ Prevention ✅ Vaccinations: Pneumococcal and influenza vaccines ✅ Avoiding secondhand smoke ✅ Breastfeeding for at least 6 months ✅ Minimizing exposure to respiratory infections in daycare or crowded settings . . . . . #AcuteOtitisMedia #Bacteria #Viruses #Otoscopy #Inflammation #FluidAccumulation

Antibiotic use and C. difficile risk: correlating prescribing with infection A new study in Infection Prevention in Practice from Bern University Hospital in Switzerland offers compelling insights into how antibiotic prescribing patterns correlate with Clostridioides difficile infection (CDI) rates across 17 clinical departments. Over 2.9 million patient-days were analyzed, revealing: -A significant association between overall antibiotic consumption and CDI incidence (no big surprise here!). - Departments like nephrology, pulmonary medicine, and haemato-oncology showed the highest CDI rates. - Specific antibiotics (carbapenems, ceftriaxone, cefepime, macrolides, and piperacillin/tazobactam) were linked to increased CDI risk. Interestingly, some departments with high antibiotic use (e.g., urology) had low CDI rates, suggesting that patient population characteristics and length of stay may also play a role (see image). The key message? Antibiotic stewardship must be tailored to departmental prescribing habits and patient profiles. Surveillance and targeted interventions will help reduce CDI rates and improve patient outcomes.
