New study in #HSR Wiley from Heather Schacht Reisinger PhD et al uses a practical approach to examining the use of Expert Recommendations for Implementing Change (ERIC) strategies by Reach, Effectiveness, Adoption, Implementation, and Maintenance (RE-AIM) dimensions for rural health innovations using annual reports on a diverse array of initiatives. WHAT IS KNOWN: Implementation science theories, models, and frameworks can support the planning, implementation, evaluation, and sustainment of evidence-based programs and practices. WHAT THIS STUDY ADDS: Content analysis of readily-available organizational documents such as annual reports can be used to examine implementation strategy utilization. CONCLUSION: Combining the dimensions from the RE-AIM framework and the ERIC strategies allows for understanding the use of implementation strategies across each RE-AIM dimension. Exploring Implementation Science: A Dive into CFIR and RE-AIM Frameworks: In the realm of implementation science, two key frameworks stand out for guiding the successful translation of research into practice: the Consolidated Framework for Implementation Research (CFIR) and the RE-AIM framework. The Consolidated Framework for Implementation Research (CFIR) provides a comprehensive taxonomy of constructs that might influence implementation success. It organizes these into five domains: 1. Intervention Characteristics - The qualities of the intervention itself, like complexity or relative advantage. 2. Outer Setting - The economic, political, and social context beyond the organization. 3. Inner Setting - The structural, political, and cultural contexts within the organization. 4. Characteristics of Individuals - The roles, knowledge, and attitudes of individuals involved in implementation. 5. Process - The activities or strategies undertaken to execute the implementation. The RE-AIM Framework, on the other hand, focuses on evaluating the public health impact of interventions through five dimensions: 1. Reach - The absolute number, proportion, and representativeness of individuals who participate in an intervention. 2. Efficacy or Effectiveness - The impact of the intervention on important outcomes, including potential negative effects, quality of life, and economic outcomes. 3. Adoption - The absolute number, proportion, and representativeness of settings and intervention agents who are willing to initiate a program. 4. Implementation - At the setting level, fidelity to the intervention, adaptations made, and the consistency of delivery. 5. Maintenance - The extent to which a program or policy becomes institutionalized or part of the routine organizational practices. Great panel yesterday with Prof. Lotte Steuten, PhD, Jason Shafrin, Melanie Whittington, and Richard Willke on the societal value of medicines, the best practices for capturing these broader elements in health technology assessments, and the implications for access and innovation in Europe and elsewhere. Earlier this month, Jason Shafrin, Melanie Whittington, Richard Willke, and nine other leading health economists published a paper in The Forum for Health Economics & Policy that suggests new methods for updating value assessments to include societal benefits. The authors identified fifteen real world values that conventional cost-effectiveness analysis omits—things like price drops due to genericization and the impact of a new treatment on caregivers and families. The paper will be an important reference for operationalizing generalized cost-effectiveness and more accurately measuring a drug’s value.
