Another Complex PCI Performed During the #حملة_شفاء_وتعافي Campaign A 65-year-old patient presented with crescendo angina. He has a known history of complex coronary artery disease and underwent multiple PCIs (RCA, IM, CX, and marginal branch) 5 years ago. Coronary angiography revealed a chronic total occlusion (CTO) of the circumflex artery (CX) due to in-stent occlusion, along with in-stent restenosis in the marginal branch. There was also a bifurcation lesion (CX/Marginal) classified as Medina 1-1-1, in a left-dominant coronary system. After several attempts, we successfully crossed the CTO segment using the #Gaia III wire and #side branch technique, followed by #anchoring and multiple balloon dilatations. Stenting was then performed, including bifurcation stenting using the #TAP technique ( unfortunately #DCB was not available) achieving a good final result.

As EtCO₂ reflects pulmonary blood flow and indirectly native cardiac output, values above 10 mmHg in VA-ECMO patients without ventricular ejection highlight significant pulmonary perfusion sustained by atrial systole.

Single-access high-risk PCI with #Impella support in a complex patient 🫀with Marco Covani, MD and Davide Donelli. 58-year-old male with new-onset heart failure (LVEF 20%). 🩻 Coronary angiography: #CTO of LCx and RCA, functionally occluded LAD with critical ostial lesion of D1 — the last remaining vessel. ▶️ PET showed extensive fibrosis but residual viability in the anterior, anterolateral, and inferolateral walls. 👥 The case was discussed with our referral #transplant center to define the best strategy for the patient. ⭕️ A heart transplant program was started. In the meantime, we performed #Impella-protected PCI with a provisional bifurcation strategy (DES on LAD–D1 and DCB on the side branch), followed by revascularization and stenting of the mid-to-distal LAD and revascularization of the proximal LCx. This case showed the quality of our regional network for advance #heartfailure, which I sincerly thanks. This study indicates that photon-counting detector computed tomography (PCD-CT) provides high diagnostic accuracy for detecting in-stent restenosis in patients with prior coronary stents. Sci-fi to triage: artificial blood is edging into care. Hemopure is available via expanded access, and PEG-carboxyhemoglobin (PP-007) just earned FDA Fast Track for acute stroke. Off-the-shelf oxygen, anyone? 🩸⚡ In 2025, U.S. centers continue using HBOC-201 when transfusion isn’t possible, while PP-007 advanced after clean safety readouts—signals that oxygen carriers may fill critical gaps in trauma and “bloodless” care. 🚑 [1][2] Caveat: older HBOCs raised vasoconstriction/MI concerns; newer nano/PEG designs aim to deliver O₂ without nitric-oxide scavenging and oxidative stress. Momentum is building across AOCs. [3] Watch: PP-007’s next trials and synthetic RBCs like ErythroMer moving from bench to bedside. Clinicians should update protocols and consent language now. A Phase III, pivotal trial of staged, bilateral magnetic resonance-guided focused ultrasound pallidothalamic tractotomy for motor complications in Parkinson's disease. Late-Breaking Abstract 21 Michelle matarazzo presents insightec study. Pallidothalamic tract (PTT) - open label 9 centres with MDS UPDRS off, off meds, UE LE primary outcomes- 40/54 did second side >6 month second side. reduction seen in scores - missing details as what parts of MDS UPDRS for each graph. Need more details about complications. FDA approved now.
