Every agent action should be logged as nodes and edges in the graph, complete with timestamps, provenance information, and outcome data. This detailed logging serves multiple purposes: it provides the data needed for continuous improvement, enables root cause analysis when things go wrong, and creates the organizational memory that compounds system intelligence over time. 

Policy integration often determines the difference between successful implementations and failed experiments. Rather than treating compliance and governance requirements as afterthoughts, successful knowledge graph implementations embed policies directly into the graph structure.
