Affidavit of Surname Discrepancy

Aizel R. Maleniza filed this affidavit to correct the spelling of his surname from "Malineza" to the proper spelling of "Maleniza" as shown on supporting documents. AFFIDAVIT TO CORRECT SURNAME DISCREPANCY

Know all men by these presents: I, Aizel R. Maleniza, of legal age, Filipino, single and with postal address at 1-7 Quinale Street, Tabuco, Naga City hereby depose and declare the following facts to correct the spelling of my Surname that is wrongly written: 1. That my true and legal Surname is spelled as MALENIZA and not MALINEZA as it appeared in my Certificate of Live Birth duly issued by National Statistic Office; 2. That the discrepancy was caused by the concerned Municipal Civil Registrar recording clerk at that time; 3. That I sought rectification of the discrepancy from the Municipal Civil Registrar Office of Donsol Sorsogon and NSO as well to establish my true identity. 4. That the erroneous spelling was eventually corrected pursuant to the decision rendered by MRC Atanacio C. Pandaan Jr. dated October 15, 2010 and affirmed by CRG under OCRG No. 10-0754518 which states that: The Childs and Fathers last names are hereby corrected from Malineza to Maleniza. Said decision to this effect is attached herewith. This affidavit is undertaken by me personally for all legal intents and purposes.
