Dear Sir, This is in reference to your passport application number BP1060355439012 dated 31/05/2012. On processing of the Application form, the following shortcomings came to notice:1) As per the PVR received you are not residing at the address given in your application. Please furnish your correct present address details with clarification. Please send three PP (Personal Particular) forms along with your correct present address details. 2) The Bonafide Student Certificate from the Institute with explanation may be furnished.

Student studying away from parents may please send three additional PP (Personal Particular) forms for verification at the educational institute along with the Bonafide Student Certificate from the Institute and also pay applicable penalty. Please submit the supporting documents in person. Please send the attested photocopies of the relevant documents only by post. The originals of such documents need to be carried while visiting Passport Office on any working day from Monday to Friday. The attestation must be done by a Gazetted Government servant. Send the documents by registered post to the address of the Passport Office mentioned above. Please make sure to mention your file number and date on top of each page/document and also on the envelope. If you live in the vicinity of Passport Office Bhopal or have been specifically asked to appear in person, you could choose to submit the required documents to the Enquiry Officer at the Passport Office on any working day from Monday to Friday between 10:00 AM and 12:00 noon.
