SAFETY ALERT
Incident Summary:

On 3rd of August at 1130hrs in JA Yard Assembly Area, a painter fell off a stepladder. He was finishing painting on a window, and lost his footing as he descended the ladder. He fell down approximately one metre to the ground, resulting in a fracture to his left elbow, requiring 14 days off work.

Stepladder work is a two-man's job one to hold and support the ladder as the other climbs up.

Contributing Factors & Lessons Learned: A ladder should only be used if there is no other reasonably practicable alternative, such as scaffolding or mobile platform. When descending or ascending on the ladder, always maintain 3 points of contact. For example, two feet and one hand or one foot and two hands. Ladders must be installed on firm and level footing. Leveling device to be used if required.

Safe work on ladder to be reminded to workers at same time when workers are instructed to do a task specially to workers doing multi-tasks where concentration is required. Toolbox Talk prior to start shift suffices but a short reminder when changing tasks is important as well. Refer to AMB Info. Sheet #22 about Safe Use of Ladder.
