A. Past Health History The patient JDV recalls that the he had a complete vaccination when he was a child. He also remembers having gone through mumps, chicken pox, and other common childhood illnesses. Patient JDV also stated that he had allergies on sea foods and no known allergy to drugs. The patient also recalls that he had appendicitis during 1st high school year 2001 and been hospitalized. It is his second time to be hospitalized as stated by the patient. He takes prescribed and self prescribed medications. He is not taking any maintenance.

B. History of Present Illness

A week prior to admission, the client experienced 40 degrees celcius of fever and weakness, difficulty of breathing as stated by the patient. The patient was diagnosed with DHF.

C. Family History of Illness Patient JDVs father was hypertensive and has arthritis. His father and mother are alive and well. Other than what was stated above, He has no known history of other diseases which may pass through genes.
