Good Day in the Name of the Lord!
I would like to in form your good office about my intention to file a vacation leave on
April 15, 2009 up to April 30, 2009.
I am currently enrolled in a review class for my forth coming NCLEX examination. This
is the reason why I am requesting for a vacation leave and Im planning to extend it until
June 2009 which will be the end of my review.
Hoping for your consideration with regards to this matter.
