JOB ORDER CONTRACT


This contract is entered by the Republic of the Philippines represented by VIOLETA B.
BOLDIOS, School Head of Inopacan Central School known as the PARTY OF THE FIRST PART and
GIOVANNI B. TIMOSAN, otherwise known as the PARTY OF THE SECOND PART both of legal age
and resident of Inopacan, Leyte.
WITNESSETH
That the PARTY OF THE SECOND PART agrees:
1. To render Security Services at Inopacan Central School
2. Accomplish the task agreed upon by both parties and submit accomplishment report to the
School Head.
That the PARTY OF THE FIRST PART will pay to the PARTY OF THE SECOND PART the amount
of Php 1,800.00 per month as School Guard from June 2015 to October 2015.
IN WITNESS WHEREOF, WE HEREUNTO affix our signatures this 1st day of June 2015 at
Inopacan Central School, Inopacan, Leyte, Philippines.
